Collection of Water Bills
1. Proceed to the CASHIER and present the issued Statement of Accounts (SOA) or if failed to bring the SOA, the Name of the registered consumer will suffice.

2. Pay the amount indicated in the SOA.

3. Ask for the Official Receipt (OR) from the CASHIER.

4. Count your change before leaving the CASHIER.

Application for New Service Connection
1. Provide complete requirements and fill up the provided Service Application and Construction Order form and Water Service Contract.

2. Applicant must pay to the CASHIER the registration fee for the New Service Connection

3. Personnel in-charge sets schedule for the installation and orients the new applicant.

4. Approval of the Head of Office (General Manager) of the application and Water Service Contract.
New Service Connection Registration fees:
(For standards 1/2" Line)
Main to Meter: Php 5,500.00
Road Crossing: Php 6,500.00


1. 3pc 2x2 photo of the Applicant with white background

2. Photocopy of any Valid ID back to back

3. Photocopy of the Community Tax Certificate or Cedula

4. Tin Number photocopy

5. Proof of Ownership (if required)

1. Fill up SERVICE REQUEST form provided by SQWD.

2. Accounts verification by the CASHIER for any balances. If found to have balance, consumer must pay the balance and the service reconnection fee to the CASHIER.

3. Approval of the Head of Office (General Manager) of the Request for Reconnection.

2. Further interview by the personnel in charge from the consumer/individual about the nature and details of the problems concerning the SQWD that need repair and maintenance.

3. Acceptance of the request the maintenance and repair by the personnel in charge.

Application for Senior Citizen Utility Discount

1. Fill up Senior Citizens Application form for discount on Water Bill form provided by SQWD.

2. Provide the complete requirements

3. Endorsement by the Approval of the Head of Office (General Manager) of the Application for SC Utility Discount.

There should be an annual renewal of the Application for Senior Citizen Utility Discount as a provision under RA 9994
1. Valid OSCA I.D. photocopy back to back

2. Proof of Billing photocopy

3. Brgy Clearance photocopy

5. Birth Certificate photocopy
